Pre-participation Athletic Physicals

Pre-participation physicals will be offered for Anthony Wayne students entering grades 7 -12 for the 2012/13 school year.

DATES:        Monday, May 14, 2012
                      Thursday, May 172012
TIME:            6:00 pm - 7:00 pm
LOCATION: St. Luke's Hospital Physical Therapy Clinic
                      900 Waterville-Monclova Rd., Suite C
                      At the corner of Waterville-Monclova Road and RT. 64
COST:          $10 payable by cash/check to AWHS
                      All money collected will be used for training supplies.
 

​PLEASE NOTE:  ALL FORMS MUST BE COMPLETELY FILLED OUT AND SIGNED BY THE ATHLETE AND PARENTS PRIOR TO THE PHYSICAL - NO EXCEPTIONS.

• Blank physical forms may be picked by from the athletic off, training room, or downloaded HERE
• No make-ups will be allowed; however, you may come either night if you are not available at your scheduled date/time.
• Girls are asked to wear sports bras under appropriate attire for the physical exam portion of their physical.
• Copies of the completed physical forms will be available upon request as no camp physicals or work permits will be completed.
